narcissistic personality disorder (npd)

Narcissistic personality disorder (NPD) is a diagnosable mental health condition defined by a pervasive pattern of grandiosity (in fantasy or behavior), a need for admiration, and a lack of empathy. People with NPD often have a fragile self-image that is carefully defended against any hint of shame or inadequacy—which is why criticism, rejection, or perceived slights can trigger rage, withdrawal, or elaborate attempts to re-establish superiority.

NPD exists on a continuum and can present in overt (grandiose) or covert (vulnerable) forms. While the diagnosis is clinical, its real-world impact on partners, children, and coworkers is profound. Trauma-informed treatment is challenging because the disorder itself often makes self-reflection feel intolerable. For those on the receiving end, education and firm self-protection are usually more realistic paths than waiting for the person to change.
